Reason for Contact

Add a reason for contact or diagnosis to a Progress Note.

About this task

Use Reason for Contact to enter a diagnosis into the patient's Progress Notes. The DOCLE-coded list of conditions is also used in Past Medical History.

Procedure

  1. In the patient's clinical record, select the Progress tab.
  2. Click Reason.
  3. In the Reason for Contact window, select the patient's reason for attending. Either:
    • Set Pick from List (Coded). In the text box, type the first few letters of the reason or condition. Double-click it.
    • In Existing Past Medical History Items, select one of the patient's previous reasons or conditions.
    • Set Free Text (uncoded). In the text box, type the reason or condition.
  4. If the reason or condition affects one side of the patient's body, set Left or Right.
  5. By default, the reason for contact is marked as Active. To mark it as inactive, unset Active.
  6. To include this diagnosis in printed letters and summaries, set Summary.
  7. To mark this diagnosis as confidential, set Confidential.
  8. To save this reason as a diagnosis, set Differential diagnosis.
  9. If you don't want the record to be saved in the patient's Past Medical History list, unset Save in Past Medical History.
  10. Click OK.

Results

The information is saved to the Reason for contact sections of the patient's progress notes.
